AXE023 08 Motor Driver board

I just received AXE023 08 Motor Driver board and I would like to see schematic of PCB to determine what connections are possible for input3.

Questions:

The two connectors labeled IN3 go where? [Obviously one goes to input3 ;-) ]
The two connectors labeled IN3/5. How are they affected by moving input jumper switch from 3 to 5?
What is 5 connected to when jumper switch is in 3 position?

See page 3 of the datasheet (click Home tab above and then datsheets).

When jumper is in left position both switches connect to input 3 (ie both switches are in parallel - either switch will activate the input).

With the PICAXE system you cannot use the jumper in the right hand position - this link is for compatibility with other programming systems.

The terminal labeled IN3 adjacent to terminal 4 and the terminal labeled IN3/5 adjacent to terminal 0 are not labeled as being connected to +V. Is this correct?

Both positions have two terminal contacts - simply connect the switch between the two contacts - you don't need any other parts.

There is a 10k pull-down already on the board for the input, so each pair of contacts provides a V+ and input pin 3 connection.

We'll add a circuit diagram into the datasheet to help clarify this.
